1. This Is NOT Medical Advice: I understand that Jake Woodard is here to serve me by sharing knowledge about health, personal, and spiritual development information. I fully understand that Jake Woodard and any of his associates do not consist of medical doctors or therapists and that, at this event, I am not receiving medical advice. Jake Woodard does not claim to cure or heal any illness or injury. I understand that Jake Woodard’s services are not offered as a substitute for clinical mental health care or medical care and are not intended to diagnose, treat or cure any mental health or medical condition.

2. REFUND POLICY: I understand that by signing up for this live workshop with Jake Woodard, I agree that I must request a refund within 7 days from my purchase before the program begins on Tuesday, October 15th.

I understand that there are no refunds once the program begins and there are no transfers or credits for this workshop.

There are no exceptions to our refund policy.

3. WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against Jake Woodard Coaching Inc., Jake Woodard, or anyone associated with Jake Woodard. I understand that this means I give up my right to bring any claims, including for personal injuries, death, disease or, or any other loss, including but not limited to claims of negligence, and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen. I further declare and represent that no promise, inducement, or agreement has been made.

4. Audio/Video Permission: I understand audio/video recording and pictures will be taken during the program with Jake Woodard. If I choose to be on camera via zoom video, I give full permission to Jake Woodard Coaching Inc. and any associates to use any audio/video that is captured during the time of the event. I give full permission for the audio/video/pictures to be used for any marketing purposes.
